The first thing I take a look at is feel. It might be the best computer mouse in the world but if it doesn't feel right in my hand I stop right there. The next most important thing for me is the feel of the buttons. I like a definite click, not sloppy or leaving you unsure if you clicked or not. For me, it's got to be wireless, I prefer the Logitech because they seem to have better range.
As far as optional buttons go, I'm not that picky. I do like forward and back buttons with a scroll as a must but the rest I'll live with.
