The first thing I take a look at is feel. It might be the best computer mouse in the world but if it doesn't feel right in my hand I stop right there. The next most important thing for me is the feel of the buttons. I like a definite click, not sloppy or leaving you unsure if you clicked or not. For me, it's got to be wireless, I prefer the Logitech because they seem to have better range.
As far as optional buttons go, I'm not that picky. I do like forward and back buttons with a scroll as a must but the rest I'll live with. After all, almost anytime you are using the computer, your hand is going to be on the mouse, comfort first and foremost, reliability next, then lastly, I go for features.
